Theory of change template
Five layers from activities to the long-term goal · A ceiling of accountability line · Numbered assumptions and the evidence behind them
Preview and use template →A logic model lays out a programme in five columns, from what you put in to the change you expect. This example covers a community food programme with staff, funding, a kitchen and school partners, running cook-and-eat sessions, food parcels, holiday meals and a budget cooking course. Coloured threads trace each activity through its outputs and outcomes, and notes below record assumptions, external factors and how results will be measured.

Write down the people, money, buildings and partnerships the programme depends on.
Outputs count what you deliver, such as sessions or parcels. Outcomes describe what changes for people because of them.
Link every activity to at least one outcome. An activity with no thread is worth questioning, and so is an outcome with nothing feeding it.
Each colour traces one chain of results from an input through an activity and output to its outcome and impact. Keep the colours when you add your own so funders can follow each thread.
