Theory of change template
Five layers from activities to the long-term goal · A ceiling of accountability line · Numbered assumptions and the evidence behind them
Preview and use template →A monitoring and evaluation framework turns a theory of change into a plan for collecting evidence. This example lists output, outcome, impact and learning indicators for a youth programme, each with a baseline, target, method and owner. Symbols across four quarters show when data is collected, reported to trustees and funders, or evaluated, and five cards set out the questions for the final evaluation.

Choose two or three indicators for each level of your theory of change. More than that and data collection will slip.
Write down where you are now and where you want to be, with the source of each number.
Mark which quarter each indicator is collected and reported, so surveys and data requests are booked in advance.
C means collect the data, R means report to trustees and funders, and E means evaluate. Move the letters to the quarters when you plan to do each task.
