Numbered process steps from goods in to dispatch · Inputs, packaging and rejects at the right step · CCP flags with limits, monitoring and actions
Preview and use template →A CCP decision tree helps a HACCP team decide, step by step, which controls are critical control points. This example draws the four classic questions down a central spine, with the "modify the step" branch to one side and every leaf showing whether the step is a CCP. Beside it, a worked table records the answers for six steps of a cooked chicken wrap.
This is a planning example, not food-safety advice or a HACCP plan. Follow your local food-safety law and an approved HACCP plan.

List each significant hazard at each process step. The tree is run once per hazard, not once per step.
Work down from Q1. Stop as soon as you reach a leaf, and write down why you answered each question the way you did.
Fill in the table so each CCP has a clear reason behind it, then carry the CCPs into your HACCP plan with limits and monitoring.
Add a row with the step and hazard, then fill in Y or N for each question as you follow the tree. Write the result and the reason for every No.
